Acne have some effetive response toward dirt, but if your body produce excess amount of sebum and and dead skin cells then this cause worse form of acne.

Acne occurs when the hair follicles become plugged with oil and dead skin cells.Hair follicles are connected to sebaceous glands. These glands secrete an oily substance known as sebum to lubricate your hair and skin.

Factors that can effect the process in worse ways are:
Hormones like androgens level get increased at puberty age and it cause more sebum production.

Some medications do cause acne.

I have never heard about chocolate for acne cause but dairy products with rich carbohydrates may cause some effects.
